Form Health — sells the wrong product for either board

We wrote a full, sourced review of Form Health before deciding it doesn't belong on our tirzepatide board or our sermorelin board. Its own /weight-loss-medications page states it prescribes only five FDA-approved brand-name medications — Wegovy®, Zepbound®, Foundayo™, Saxenda®, Contrave® — filled through ordinary insurance or the pharmacy of a patient's choice. No compounded product appears anywhere on its site. Both PeptideRank boards are explicitly, repeatedly branded as compounded-only, and each board's own honesty banner says outright that what it ranks is “not Zepbound, the FDA-approved brand.” A Form Health row would make that sentence false, so it doesn't get one.

That is not a knock on Form Health's program — for a reader who wants an FDA-approved brand-name drug billed through insurance rather than a compounded formulation, it is a genuinely different, legitimate kind of service. It simply isn't the kind of service either board here ranks, and we would rather say that plainly than stretch the scope of a “compounded providers” board to fit a company that sells the opposite of what the board promises.

The review itself has no live page today. It is complete and dated like every other entry in our provider-review system, but a review needs a board or a paid destination to resolve to before we publish its URL — Form Health has neither right now — and we do not publish a link we know returns a dead page. It is held, not deleted, for the day a brand-name/insurance board exists to measure it against.

GOAL.MD — couldn't be checked at all

GOAL.MD came up as a candidate the same way every provider on either board did — worth an independent look. When we went to verify it directly, its live site was suspended by its own host's Trust & Safety team. We confirmed that finding a second way, through the Wayback Machine's archived snapshots, rather than taking a single dead page at face value.

A suspended site is not something we can verify a price, a pharmacy claim, or a state list against — every fact on either board is read directly off a provider's own live page, on a dated check, and there was no live page to read. So GOAL.MD was never added to a board, and no review was written for it: research stopped at the point where there was nothing left to independently check, before it reached the stage of writing a verdict.

Why this page stops at two

Two is the honest count today, and we'd rather publish a short, accurate page than pad it with candidates we haven't actually researched to a conclusion. As other candidates go through the same process — real research against the provider's own site, a decision reached, a reason written down — this page grows with them.
