The catch — The advertised price is the real price with the shipping fee subtracted. Its store page quotes $127.90 plus $38 refrigerated shipping; its pricing page lists $165.90 and states that all prices include shipping. The two pages disagree, and the advertised headline is the one that omits a mandatory cost. Its separate “from $34 a week” claim reconciles with nothing on the price table at all.

“Starting as low as $127 per month” on the semaglutide store page, and “Weekly doses start at $34 for eligible patients” on its weight-management page.
What you actually pay
$165.90 per four-week supply at the lowest semaglutide dose, rising to $355 at the top of the ladder. Tirzepatide runs $210 to $434, and the sublingual versions $164 to $460. Sermorelin is $179 per 30-day supply and NAD+ nasal spray $165. Note the unit: four weeks, not a calendar month, so thirteen fills a year.

- The clinician visit is billed separately — $250 initial and $150 for continued care as a self-pay patient, or a copay and deductible with insurance. Midi advertises no membership fee, which is accurate but easy to read as no fee beyond medication.

The bottom line
Midi Health (joinmidi.com) is a menopause and perimenopause practice that also sells cash-pay compounded GLP-1s, sermorelin and NAD+, with the clinical visit billed to insurance and the medication paid in cash. It publishes its full dose ladder, which is more than most manage, and its outcome claim carries an unusually complete methodology footnote. The defect is in the headline. Its semaglutide store page leads with “Starting as low as $127 per month” and then concedes, further down the same page, that the figure is $127.90 plus $38 for refrigerated shipping. That sum is $165.90 exactly — which is the lowest number on its official pricing page, a page whose stated premise is that all prices include shipping. One page strips the shipping out and only the stripped version is advertised.

Who it is for, and who it is not
A good fit if
- You want the whole dose ladder published before you commit — every tier for both compounds and both routes is listed, which most competitors will not do.
- Your insurance covers the visit: the clinical encounter is billed to insurance while the compounded medication stays cash-pay, an unusual split that can work in your favor.
- You want an outcome claim you can actually assess — its 16 lbs average is footnoted with the cohort size, the date range, and the admission that weights were self-reported.
Look elsewhere if
- You budget from the advertised figure. No customer can pay $127.90; the real entry price is $165.90 per four-week supply.
- You are on Medicaid or Medi-Cal. Midi states it cannot treat you at all, explicitly including as a self-pay patient.
- You are titrating up. Prices more than double across the ladder — $165.90 to $355 on injectable semaglutide, $210 to $434 on tirzepatide — and no annualized figure appears anywhere.

What its own site says
A category with no name and a name with no category, on separate pages. Its pricing page states its partner pharmacies are licensed and 503B-accredited while naming none — and that phrasing is itself wrong, since outsourcing facilities register with FDA rather than being accredited. The only named compounding pharmacy on the property appears in the Terms and carries no category. The claim also sits awkwardly against what Midi sells: patient-specific microdosing across a fifteen-step ladder is not what a standardized outsourcing facility produces.
What's the difference between 503A and 503B?
A 503A pharmacy compounds for one named patient; a 503B outsourcing facility manufactures in batches under CGMP. Neither is a quality guarantee on its own — a named facility is simply a fact you can check, and an unnamed “partner pharmacy network” is not. Compounded peptides are not FDA-approved.
Where you can get it

Its marketing claims all 50 states, but that claim is about insurance and employer channels rather than clinical availability. The only enumeration it publishes lists roughly 21 states, and its Terms concede availability is limited to states where Midi operates without listing them. The part they would rather you skipped
Its self-pay prices ship hidden. On the pricing page the widget opens on the Insurance tab, which shows only that you pay your copay and deductible — no dollar figure at all. The actual cash prices, $250 for an initial visit and $150 for continued care, sit in a second panel that ships with the hidden attribute set and appears only if you click Self-Pay. A reader who does not click sees no price.
