NAD+: what it is, what it costs, and who sells it

NAD+ is a coenzyme present in every cell, central to energy metabolism and DNA repair — a real, well-established piece of basic cell biology, and the reason “boosting NAD+” is a coherent hypothesis rather than a fabricated one. A live DailyMed search for “NAD” (93 results) returns no FDA-approved NAD+ drug product of any kind — the results are Nadolol (an unrelated beta-blocker), homeopathic combination remedies, and over-the-counter cosmetic listings.

What actually gets sold under the NAD+ name splits into products with genuinely different evidence records: oral precursors (nicotinamide riboside, nicotinamide mononucleotide) that raise NAD+ indirectly, and injectable, IV, or nasal-spray NAD+ itself — the product this site's board of 38 ranked sellers overwhelmingly sells. Those two literatures are not interchangeable, and blending them is exactly what most marketing does.

A 2026 systematic review of 113 eligible studies (33 human, 80 rodent) evaluating NAD-related compounds found, in its own words: “No eligible outcomes trials evaluated intravenous or intramuscular NAD⁺ itself for anti-aging or wellness indications.” Zero. The one human pharmacokinetic pilot of directly infused NAD+ didn't even detect a plasma rise until after two hours of continuous infusion. Oral precursors are a different story: 28 randomized human trials consistently show they raise blood NAD+-related metabolites, though effects on functional outcomes are mixed — and that is a genuinely different, and commercially separate, product from what most of this board's rows sell. NAD+ is a naturally occurring coenzyme, not a novel synthetic peptide under FDA Pharmacy Compounding Advisory Committee (PCAC) review — unlike BPC-157, TB-500, and GHK-Cu, it does not appear on FDA's 2026 PCAC meeting agendas, because it sits in a different regulatory category rather than an earlier, unresolved stage of the same review. A retrospective real-world tolerability study from a commercial IV-therapy chain found that patients receiving NAD+ IV reported moderate-to-severe gastrointestinal symptoms, increased heart rate, and chest pressure during infusions — a genuine, documented infusion-related safety concern that is specifically an infusion-rate issue, not a dosing-arithmetic one.
