Not many sellers sit near those floors. Only 3 semaglutide sellers and 3 tirzepatide sellers charge within $25 of them. The middle of the market is much higher: among sellers with a plain monthly rate, the median is $199 for semaglutide and $277 for tirzepatide.

Is compounded semaglutide or tirzepatide cheaper per month?

Semaglutide, by a wide margin. Its floor is $40 lower and its median is $78 lower. Of the 81 sellers with a plain monthly rate for both drugs, 77 charge more for tirzepatide.

Why is the "$X first month" price not the real price?

Because a first-month price is paid once. Of the 589 advertised GLP-1 and peptide prices we record, only 342 (58%) are one plain monthly rate. The rest are a first month that rises, a "from" price, a per-month figure that needs a prepay, or a price plus a required fee.

That is why the lowest advertised figures sit below the floors above. The lowest on the semaglutide board is $49 — only a "from" price, from WellMedR. On tirzepatide it is $84 — only a "from" price, from RxPepsDirect. Our price transparency page sorts every advertised price by shape.

Yes, but on narrower grounds. FDA declared the tirzepatide shortage resolved (October 2, 2024, confirmed December 19, 2024) and the semaglutide shortage resolved on February 21, 20253. The grace periods for compounders have ended.

A state-licensed pharmacy can still compound for an individual prescription. It cannot regularly make what FDA calls "essentially a copy" of an approved drug. FDA says that rule does not apply when a prescriber documents a change that makes "a significant difference" for that patient3. Read the details for semaglutide and tirzepatide. A compounded drug is not FDA-approved and is not the same as the brand.

What else should I check besides price?

Three things, and each one can matter more than a $20 gap:
